Each sheet covers one roll. The header captures the roll's identity; the table captures every frame.

| Roll name | Something you'll recognise later — "Portra, Leica, city walk". |
| Camera / Lens | Which body and glass — the "which roll is in which camera" answer. |
| Film stock / ISO · EI | The film and the speed you rated it at (note push/pull here). |
| Format | 35mm, 120, large format, and so on. |
| Aperture / Shutter / Exp. ± | Your exposure for each frame, plus any compensation from the meter. |
| Lens / Location / Notes | Focal length, where you stood, and anything worth remembering. |
| Keep | A quick tick for frames you think are keepers. |
How to use it
- Print one sheet per roll (or keep the CSV open on a laptop).
- Fill in the header when you load the film.
- Note each frame's settings as you shoot — while you still remember them.
- Keep the sheet with the roll until it's developed and scanned, then match frame numbers to your scans.
